The Patient Centered Medical Home and Alaska Primary Care Association Fall Summit will be taking place from September 11-14 in Anchorage, AK. The conference will focus on the national move towards a Patient Centered Medical Home, with the goal being to provide the resources to initiate and optimize that transformation.
